Services are tailored to your unique needs can be provided in the comfort of your own home, in care facilities or from our offices across Perth and in the South West. We also travel to regional and remote areas, including the Wheatbelt, Mid West and Great Southern.
Our Aged Care Services
Occupational Therapy
Live life to the fullest with support to ensure you can complete daily tasks and do the things that are important to you. We also provide support for dementia mangement and falls prevention.
Physiotherapy
Maintain your movement, strength and mobility and maximise your functional wellbeing. Services include the provision of exercise plans if needed.
Assistive Technology
Prescription of assistive technology, aids and equipment that can maintain and improve your independence and safety.
Home Modifications
Advice, support and prescription of modifications and aids to improve the safety, accessibility and comfort of your home.
Dietetics
Advice and support for diet and nutrition to help maintain your health and wellbeing. This can include the development of nutrition plans.
Speech Pathology
Access support if you have difficulty communicating or swallowing.
Continence Management
Our Bladder and Bowel Health Team comprise experienced continence nurses and physiotherapists who provide the best in continence care. Services include thorough bladder and bowel health assessment, continence management strategies, trial and prescription of incontinence products, subsidy scheme access, GP reports, education and training.
